Baptist Health Home Health and Hospice Disease Management Programs

Baptist Health Home Health and Hospice has been positioned well for the future by designing and implementing “Disease Management Programs” for populations at risk for re-hospitalization.

Baptist Health Home Health and Hospice, by our very nature, have had most of the primary components of Disease Management in place.  Our staff focus on patient education and instruction in patient self management, as well as collaboration with the primary physician.  Our Home Health Agency employs experienced registered nurse, physical therapists, occupational therapists, wound care specialists, and dietitians to ensure that all patient needs are met. 

Recently, our agency has incorporated the use of evidence-based guidelines for care which have been embedded in our electronic documentation system to prevent practice variation.  Critical pathways have been developed for heart failure, diabetes and high risk pregnancy management.  Reinforcement of teaching and patient follow-up is augmented by the use of our telehealth system.  These monitors are placed in the homes of high risk patients so that pulse rate, weight, blood pressure, pulse oxygen levels, body temperature and glucose levels can be monitored every day.

Our agency is outcome driven and makes heavy utilization of OASIS data to formulate improvement initiates that bring results. 

Evidence of our positive health outcome achievements can be seen on the Home Care Compare web site, (www.medicare.gov)  as our agency has scored better than state and national averages in eight out of ten important measures (for example, our recent re-hospitalization rate is at 20% versus 28% for the nation).
